Nothing you enter reaches us. Patient answers, signatures and finished forms are stored on your device. We have no copy, so there is nothing on our side to breach, subpoena, or lock you out of.

Purchasing is not open yet. The licensing service is deployed but not yet switched on. Until it is, join the mailing list and we will tell you when it opens.

This licence belongs to the physician you named. You can cancel any time. Their access runs to the expiry date printed on their most recent licence — about a year out — and then stops.

Their licence and every notice about it go to this address. It is echoed back below before any money moves, so a typo is fixed here for free.

I am a licensed healthcare provider — or authorized staff purchasing on behalf of one — and I agree to the Terms of Service and Privacy Policy.

Why we ask: these confirmations protect your patients and make clear the clinical judgment on every form is yours — the tool never signs anything.

Can I email you occasionally to ask how it's going? (optional)

Cancelling. Cancel any time from the subscription-management page linked in every receipt Paddle sends you, or by emailing support@patientpapers.app. Cancelling stops the next charge; the licence already held runs to its printed expiry date.

Getting the licence. Payment mints a licence key for the provider you named. It is shown on the next screen and emailed to them, and it does not expire. Each period they use it on the licence page to get a fresh licence, and we send one by email too.

Renewal reminder. Before an annual renewal we email at least 30 days in advance, stating the price that will be charged.

Payment. Our order process is conducted by our online reseller Paddle.com, the Merchant of Record for all our orders. Paddle collects the card; we never see it.
